According to preliminary findings based on her report, Ms. Safo suffered a gunshot wound to her left ear while visiting her late father’s residence. She subsequently reported the incident to the Kwabenya Police, presenting a vehicle with registration number GN 3482-24, which displayed visible gunshot damage. Police have confirmed issuing her a medical report form, and she is currently receiving treatment. The vehicle has been retained for forensic examination as investigators work to establish the full circumstances and identify all persons connected to the case. Further reports suggest the incident occurred near her brother Israel Kwadwo Safo’s residence, with approximately 15 rounds of ammunition allegedly discharged, and one bullet grazing her near the neck.

Speaking publicly, Kantanka alleged that Sarah Adwoa Safo was, in fact, the individual who fired the first shot during an event organized to formally introduce the church's newly appointed leader to its General Council. He detailed that Ms. Safo arrived at the venue and made repeated attempts to force her way through the gates with her vehicle, first at the southern entrance and then at the northern gate, causing loud bangs and disruption.

According to Mr. Kantanka, private security personnel on duty became gravely concerned that Ms. Safo's aggressive actions could endanger the numerous individuals gathered within the premises. He stated that the information he received indicated Adwoa Safo fired the initial shot before physically ramming her car into the gate, which then prompted the private security to fire warning shots to prevent further danger and potential casualties. Kantanka was emphatic in his assertion that, contrary to initial reports, no one was actually shot at the event. He also categorically rejected suggestions that Adwoa Safo had been barred from entering, emphasizing that she would have been allowed access had she adhered to normal entry procedures, such as parking and proceeding on foot.
