Ruth's death came weeks after she was involved in a devastating accident. On Wednesday, August 13, 2025, she was hit by a heavy-duty truck, reportedly linked to the Dangote Group, near Auchi Polytechnic in Edo State. Eyewitnesses at the scene recounted the harrowing incident, stating that the truck crushed her leg before a bystander managed to intervene and stop the vehicle. The impact of the accident left her with severe leg injuries, which unfortunately necessitated an amputation. This tragic event occurred barely six days after Ruth had celebrated her graduation from Auchi Polytechnic, turning a milestone achievement into a period of profound grief for her family.
In the wake of her passing, numerous Nigerian celebrities and public figures have extended their heartfelt condolences to Phyna and the Otabor family. Nollywood actress Kate Henshaw expressed her sorrow, writing, "Ehhh God, May God comfort your family at this difficult time." Fellow Big Brother Naija alumnus Wanni also shared deep pain, commenting, "Oh no! I'm so pained, No! No! Ruth went through too much pain, may her soul rest in peace." Media personality Hero Daniels offered his sympathies, stating, "My sincere condolences, May God grant you and your family fortitude to bear this great loss. RIP Ruth." Similarly, reality TV star Daniella Peters conveyed her condolences, saying, "Kai, so sorry Phyna, My Condolences."
