Crisis Deepens: UK's Blood Supply Dips to Critical Levels, Patient Lives on the Line

Despite these advancements, the NHS faces a critical shortage of blood, urgently needing more donors, especially those of Black heritage, to meet the growing demand for this vital treatment.
Caroline Fyneface, known as Caz, was just 13 when she experienced her first severe sickle cell crisis, describing the pain as frightening and treatments ineffective at the time. Diagnosed with sickle cell disorder through the NHS Newborn Blood Spot Screening Programme, or 'heel prick test,' Caz experienced few symptoms in early childhood. However, her first major crisis led to frequent hospital admissions, profoundly impacting her education, friendships, social life, and mental well-being.
A turning point came in January 2024 when Caz began regular red cell exchange treatment. Every seven weeks, she receives seven to eight bags of donated blood, replacing her sickled red blood cells with healthy ones. Thanks to these transfusions, 23-year-old Caz has successfully completed a Law with Criminology degree at Oxford Brookes University and is now preparing to qualify as a solicitor in Manchester. She credits the heel prick test for her early diagnosis, which, though not discussed much during her childhood, ultimately led her to life-changing treatment. Caz notes that since starting regular blood exchanges, her hospital admissions have become less frequent, crises are better controlled, and her overall quality of life has significantly improved.
This year marks 20 years since newborn screening for sickle cell disorder was introduced in England. This crucial programme has transformed outcomes by enabling earlier diagnosis and access to specialist care, leading to a growing population of over 19,000 people now living with the condition. Before universal screening, many children were only diagnosed after experiencing serious, often life-threatening complications. Since the early 2000s, early diagnosis has shifted this outlook, identifying babies before symptoms develop and facilitating prompt support and treatment.
Furthermore, changing treatment guidance now emphasizes proactive full blood exchanges or transfusions to reduce and manage painful 'crises,' rather than waiting until a patient is in severe pain. While these advancements in screening, specialist care, and treatment have helped more children born with sickle cell survive and thrive into adulthood, they have also led to a sharp increase in demand for closely matched blood, which many patients rely on.
Currently, the NHS can only meet approximately half of the blood demand for patients with sickle cell. Gerry Gogarty, director of Blood Supply at NHS Blood and Transplant, states that there are more people with sickle cell reliant on regular blood transfusions than ever before, and the demand is growing faster than supply. While there has been an incredible 120% rise in Black heritage donors over the past decade, an urgent need for at least 16,000 additional Black heritage donors remains to meet current demand, requiring a three-quarters increase in the current donor base.
There is a particular need for donors of Black heritage because they are 10 times more likely than white people to possess the Ro blood subtype, which is vital for treating many people living with sickle cell. Iyamide Thomas, of the Sickle Cell Society and NHS engagement lead for the screening programme, highlights the significant progress of the last two decades but stresses that these life-saving treatments depend on a reliable supply of closely matched blood. She urges more people of Black heritage to come forward, emphasizing that regular donations directly help people with sickle cell lead healthier, more independent lives.
To facilitate donations, NHS Blood and Transplant has introduced several improvements, including extra community sessions, ongoing enhancements to HB (iron) testing, and priority access to appointments for all registered donors of Black heritage.
