She is the only one complaining about it. Many other people cannot initially hear me when I first answer their calls.

If she’s the only one complaining, the problem is in her VoIP phone or provider. How long is “initially”? There can be a 1 or 2 second pause before the network sees that you have answered.

Yes, that's what I think too. But, since she is living in a senior home, she doesn't really know what she's got. I am just guessing that they are on VOIP. It's a cheap way to go.

Well, only about 4 or 5 seconds. Still, it is annoying.

I understand, but it is a problem with the network, not your iPhone. Are you using WiFi calling? Try turning it off. Or if it is off try setting it up..

pretty much all “landline” phones are VoIP.

The last possibility I can think of, and Perhaps a long shot:

Do you have a 3rd party VPN installed, even if you are not using it? If so, delete the VPN profile in Settings/General/VPN & Device Management/VPN, then restart your device. Don't just turn it off; delete the profile. If that fixes it, you can try reinstalling VPN, but probably shouldn't.

When on speakerphone, I do hear from the top speaker to which I lean against my right ear. When speaking to a person out of the country (or foreign immigrants), I can barely hear them even on speakerphone.
