Kaiser Permanente in Northern California faced a significant one-day strike on Wednesday, involving approximately 2,400 mental health professionals and more than 23,000 nurses. The striking mental health professionals, including social workers and psychologists, provide crucial mental health and addiction medicine treatment for an estimated 4.6 million patients across the San Francisco Bay Area, central valley, and Sacramento regions. The primary catalyst for the widespread labor action centered on profound concerns that the healthcare giant might be moving towards replacing human therapists with artificial intelligence (AI), alongside other critical issues impacting patient care.

The National Union of Healthcare Workers, representing the mental health professionals, voiced fears that while Oakland-based Kaiser Permanente does not currently employ AI for therapy, the technology's rapid advancement could soon make it an appealing option for the company to use as a substitute for human practitioners. This apprehension forms a core part of the ongoing negotiations for a new contract, which have been underway since last summer.

Kaiser Permanente, however, vehemently disputes the union's claims, asserting that AI will not be used to replace human assessment or to make care decisions for patients. In a message sent to employees, management, including Lionel Sims, senior vice president of Human Resources, and Priya Smith, chief employee human resources officer, stated that they view "technology — and AI, in particular — as a way to support you in managing your practice and provide you with tools that facilitate greater access to care and connection with patients." The company also affirmed that its facilities remained open during the strike and noted an increase in mental health worker hires.

Beyond the AI debate, the union is also pushing back against management's demands to curtail the time allocated for patient notes and responding to patient messages. Dr. Emma Olsen, a psychiatrist at Kaiser in Vallejo and a union steward, articulated the union's stance, explaining, "They’re trying to take all that time away. They really just want us to be seeing people back to back to back, to be seeing more people for less time with less resources." This demand, according to the union, would detrimentally impact the quality and thoroughness of patient care.

The concerns raised by mental health professionals resonated deeply with the California Nurses Association. Katy Roemer, a nurse in adult and family medicine, expressed the shared sentiment, emphasizing the importance of human-to-human care and posing critical questions regarding AI's ultimate beneficiaries. She asked, "Is AI going to benefit patients? Is AI going to benefit the people that work for Kaiser Permanente? Or is AI going to benefit the bottom line of the corporation? So we want AI that’s transparent, that is allowing people to do their jobs." This highlights a broader demand for AI implementation that prioritizes patient and worker welfare over corporate profit.

This labor dispute occurs in the context of previous challenges for Kaiser Permanente regarding mental health services. In 2023, the organization agreed to a substantial $200 million settlement with the California Department of Managed Health Care following violations of state mental health laws. This history underscores the existing pressures and scrutiny surrounding Kaiser's provision of mental health care, adding another layer of complexity to the current negotiations and strike.
