Assessing Breast-Specific Body Image Concerns in a General Population of Women: Introduction of the Breast Appearance Concerns Scale

Body image dissatisfaction, particularly related to breast appearance, plays an important role in cosmetic breast surgery (CBS) decisions and psychological wellbeing. However, existing measures often overlook healthy women considering CBS and fail to adequately address the nipple-areola complex (NAC), a critical component of breast satisfaction. This study introduces the 12-item Breast Appearance Concerns Scale (BACS), a psychometric tool designed to fill these gaps. Data were collected from a diverse sample of 589 female young adult women who completed the BACS in addition to measures of constructs, such as self-esteem and anxiety, which have been associated with body image. Exploratory and confirmatory factor analyses supported a two-factor structure: NAC satisfaction and general breast satisfaction. The BACS demonstrated strong internal consistency (α = .785) and test-retest reliability (r = .741). Predictive validity analyses revealed that the general breast satisfaction subscale effectively distinguished individuals who had considered CBS from those who had not (classification accuracy = 72.1%). While the NAC subscale was robust in its structure and reliability, its predictive utility was limited in this sample, warranting further research in diverse populations and clinical contexts. These findings position the BACS as a promising tool for assessing breast-specific body image concerns, particularly general breast satisfaction, with potential applications in clinical care and research on body image and CBS decision-making.
