Conor Gallagher tied the score to make it 2-2 on aggregate in just over 30 seconds, with his strike proving to be the only goal of regular time amid Vinicius Jr. missing a penalty.
In the shootout, Julian Alvarez appeared to have scored yet had his kick overturned by VAR which deemed him to have taken a double touch.
Madrid then ran out 4-2 winners after Marcos Llorente also missed for the hosts in the Metropolitano and Toni Rudiger converted the deciding kick to save Lucas Vazquez’s blushes.
And while head coach Carlo Ancelotti was adamant that Alvarez kicked the ball twice, Simeone wasn’t having any of it after being shown a clip of it by Movistar prior to heading to his post-match press conference.
“Have you seen it?” Simeone asked journalists himself in the room after the topic was raised. "What did you see? Did he touch it [twice]? Don’t be afraid.
“Did you see it? What did they see? Did he touch it? Don’t be afraid. Did anyone here see that Julian touched it twice. Raise your hand. Another question. I didn’t talk to Julian yet,” Simeone added.
Earlier in his press conference, Simeone refused to add that past shootout losses to Madrid - in the 2016 UCL final for example - weighed heavily on his men.

Oblak yes, [Angel] Correa, [Marcos] Llorente in the Super Cup… But it means nothing. We played a very good game, we competed in an incredible way, we were able to score and force extra time.
“We got to penalties … I just saw the image and the referee says that when he steps on the penalty spot Julian puts his foot down but the ball does not move. I’m proud of my players, I’m happy, we competed in an exemplary way.
"We haven’t been able to beat Real Madrid in the Champions League, of course, but they had a hard time every time. They’re going to remember enjoying having beaten us, but when they’re alone they’ll think they’ve always had it hard with us,” Simeone insisted.
